Insurances Generally Accepted at the Raby Institute

Raby Institute participates in a variety of insurance plans and is considered an “in-network” provider for a large number of insurers. Being in-network means that insurance companies have made special contracting arrangements to provide a higher level of payment coverage for their customers who receive care at Raby Institute. If Raby Institute is part of your insurance network, your out-of-pocket expenses for care at our facilities may be reduced. Insurance coverage changes. Please check with your insurance company regarding insurance coverage prior to your visit.

What is your Financial Responsibility?

Your insurance coverage will vary based on the specific plan you have chosen, Please check with your insurance provider to understand your specific benefit coverage and your financial responsibility.

Below are some key components of insurance plans that you may want to be familiar with to help understand your financial responsibility.

Co-Payment

A flat fee amount that you are required to pay at the time of service.

Deductible

The amount that you must pay EACH YEAR before your insurance will begin paying your medical claims.

Co-Insurance

The percentage of your medical bill you will be responsible to pay once you have met your deductible.

Out of Pocket Maximum

The maximum amount that you will pay out of pocket for medical expenses per policy year.

What to Expect at Check-In

When checking in at the Raby Institute, you will be asked to pay any applicable co-pay and any balance on your account.  If you are seeing one of the treatment providers you will also be asked to pay any applicable deductible.  

All services that are fee for service will be payable, prior to services, at the time of check in.

A credit card on file is required to establish care at the Raby Institute.  If one is not on file you will be asked to provide a card to be kept securely on file.  Per the financial agreement, your card will be used to process any balances over 60 days. 

When patients do not attend their appointment, we are unable to fill that space. Therefore, a no show fee of $25 will be charged if you fail to show up to your appointment without prior notice. A 3 strike policy is in place for a cancellation or reschedule within 24 hours of the scheduled appointment. On the 3rd strike a $25 fee will be issued and each subsequent violation will result in a $25 fee.
